OSHR Procedure for Reporting State Employee Fatality, Amputation, Loss of Eye(s), or Hospitalization Immediately Following Injury

It is important to report any and all accidents that occur in your workplace. However, for injuries that result in fatality, amputation, or hospitalization involving state, contract, or temporary employees, the following action steps should be completed immediately.
