Protecting your sight is important—that's why the NCFlex Vision Plan offers employees the Core Wellness Exam option at no cost if they enroll.
For those looking for more comprehensive coverage, the Basic and Enhanced options offer additional exam and materials coverage and allow employees to cover dependents. The rates, frequency at which you can get frames and the frame and lens allowances are based on the specific plan option.
Understanding Each Option
Cost:
Employee Only: $0
Employee and Family: N/A—Employees cannot add dependents to this option of the Vision Plan.
The Core Wellness Exam option is available at no cost to employees as long as they enroll.
With the Core Wellness Exam option, you can receive an annual comprehensive eye exam for a $20 copay. If vision materials are needed, there are lens allowances and discounts on frames from providers in the EyeMed network.

Employee Only: $4.50/month
Employee and Family: $11.66/month
The Basic option provides:
- An annual comprehensive eye exam
- $120 allowance for eyeglass lenses or contact lenses once every 12 months (per covered person)
- $125 allowance for frames once every 24 months (per covered person)
You may purchase additional materials at a discount.
Cost:
Employee Only: $8/month
Employee and Family: $20.52/month
The Enhanced option provides:
- An annual comprehensive eye exam
- $175 allowance for eyeglass lenses or contact lenses once every 12 months (per covered person)
- $200 allowance for frames once every 12 months (per covered person)
You may purchase additional materials at a discount.
Need to Know Details
The Core, Basic and Enhanced options of the Vision Plan offer in-network and non-network benefits. Employees pay less when they use a network provider. The employee is responsible for paying any charges in excess of the covered benefit.
When using a non-network provider, you pay the provider in full and submit an out-of-network claim form with a copy of the receipt to EyeMed. After doing so, you can be reimbursed up to the amount of the out-of-network allowance.
There are more than 5,051 providers in the state, including independent eye doctors, retail stores and even online options. If your vision care provider is not part of the EyeMed network, you or your provider may contact EyeMed with the provider's name, address and telephone number to begin the provider nomination process.

Be aware that some eye doctors and retailers may run special promotions stipulating that participants not use their benefits to take advantage of special pricing. When considering a purchase, employees should talk with their provider about their options or call EyeMed at 866-248-1939.
Employees can access members-only special offers online or by downloading the EyeMed Members App.
No benefits will be paid for services or materials connected with or changes arising from:
- Orthoptic or vision training, subnormal vision aids and any associated supplemental testing; Aniseikonic lenses.
- Medical, pathological or surgical treatment of the eye, eyes or supporting structures.
- Any vision examination or any corrective eyewear required by a policyholder as a condition of employment; safety eyewear.
- Services provided as a result of any Workers' Compensation law, or similar legislation, or required by any governmental agency or program whether federal, state or subdivisions thereof.
- Plano (non-prescription) lenses.
- Non-prescription sunglasses.
- Two pairs of glasses in lieu of bifocals.
- Services or materials provided by any other group benefit plan providing vision care.
- Services rendered after the date an insured person ceases to be covered under the policy, except when vision materials ordered before coverage are delivered, and services rendered to the insured person are within 31 days from the date of such order.
- Lost or broken lenses, frames, glasses or contact lenses will not be replaced except in the next benefit frequency when vision materials would next become available.
In addition to their coverage, Vision Plan members have access to special offers including discounts on frames and lenses, savings on contacts, exclusive offers from network providers and retailers, free shipping from online retailers, special pricing on lens cleaner and glasses cases and much more.
Simply register online to review the latest discounts and give your benefits a boost. Employees may receive:
- 20% off any remaining cost for frames once the frame allowance has been applied.
- 40% off unlimited, additional complete pairs of prescription eyewear.
- 15% off any balance over the conventional contact lens allowance.
- 20% off any item not covered by the benefit.
- Discounted set pricing on hearing aids and free batteries.
If you terminate employment with the State of North Carolina, you may continue your EyeMed vision coverage by paying the monthly premium directly to the COBRA vendor. When an employee retires or otherwise leaves employment, COBRA enrollment materials are sent to the employee's last known address.
During annual enrollment, an employee may change between the Core, Basic and Enhanced options.
If applicable, the frame allowance will change each calendar year depending on the option of the Plan an employee enrolls in.
An employee may enroll in only one of the three vision coverage options. To receiver family coverage, you must enroll in the Basic or Enhanced option.
Tools to Make the Most of Your Vision Plan
Member Web is your Vision Plan control center—a place to manage the details of every visit and every claim.
To manage your benefits in Member Web:
- Visit eyemed.com and click on Member Login.
- If you're a new user, click on Create an Account.
- Register using your member ID or the last four digits of your social security number depending on how your benefits administrator entered you into the system. Then you'll get an email asking to confirm your account.
- Finish setting up your new account with your email address and a secure password.
- Begin using Member Web to view your benefit details, confirm eligibility, check claim status, print replacement ID cards and more.
On the go? Your benefits can go with you. EyeMed Mobile App helps members get the most from their EyeMed experience—anytime, anywhere.
Check out the App Store or Google Play to download the app. From there, members can:

With the tool, you can estimate your out-of-pocket costs, find simple definitions and interactive examples of common products and add-ons and more.
Members can follow the directions to use the tool:
- Register or log into your account at member.eyemedvisioncare.com and click the Estimate Costs tab.
- Select the service you want an estimate for.
- Choose the frame type you're looking for and explore a variety of options with details on each product.
- Get a clear summary of your estimated out-of-pocket costs based on your selections.
