The NCFlex Cancer Plan is administered by Allstate Benefits and offers three plan options, Low, High and Premium. 

• This plan pays out benefits directly to you for medical treatment resulting from cancer or 29 other specified diseases.  Learn more about the plan, what's covered and what payouts are available.  
• All three options feature a Wellness Benefit that pays you money for completing one eligible screening per year per covered person.  
• Be sure to designate beneficiaries in the enrollment platform. 
• There is no deadline to file claims and documentation IS required.
  • Cancer claims can be filed online or with a paper form. View and print the cancer claim form or the wellness claim form.
