Cancer and Specified Disease Plan

The Cancer and Specified Disease Plan pays cash benefits for cancer and 29 other specified diseases to help with the costs associated with treatments and expenses as they happen. This coverage can help pay for hospitalization, surgery, radiation/chemotherapy and more. 

You can choose between three options on the Cancer and Specified Disease Plan: Low, High or Premium. All three options cover the same type of services. In most cases, the amount of coverage differs based on the option chosen.

Each plan option includes the Cancer Prevention and Screening Benefit, which pays a benefit for each covered person annually for taking one of the tests below, regardless of the cost of the test.

  • CA125 (cancer antigen 125 - blood test for ovarian cancer)
  • CA 15-3 (cancer antigen 15-3 - blood test for breast cancer)

  • Doppler screening for peripheral vascular disease

The Cancer Plan includes a benefit that pays you directly when you travel more than 70 miles from home for treatment of a covered diagnosis, with a maximum benefit amount equivalent to 700 miles.

Allstate Benefits does not pay benefits for a pre-existing condition during the 12-month period beginning on the date coverage starts. 

A pre-existing condition is a disease or physical condition for which medical advice or treatment was received by the covered person during the 12-month period prior to their effective date of coverage. 

Any covered loss that is incurred after the first 12 months is payable. 

The policy does not pay for any loss except those due to cancer or a covered specific disease. A diagnosis must be submitted to support each claim.

Evidence of Insurability (EOI) is a way to provide proof of good health. This may include the employee's current health status, medical history and family history. 

If you are required to submit EOI, Allstate Benefits must approve EOI before coverage becomes effective. The EOI form is available here; only complete the form if you are instructed to do so. 

New hires do not need to provide EOI if they enroll within 30 days of their date of hire. 

The portability feature allows employees to continue cancer coverage when their employment ends or policy terminates by paying premiums directly to Allstate Benefits.

For individuals eligible for Medicaid, this plan may not be the best choice. 

Benefits assigned under the policy are required to be assigned back to Medicaid. 

When you enroll in NCFlex's Cancer Plan, be sure to designate beneficiaries in your enrollment platform

If you do not designate a beneficiary when enrolling, you can complete the Cancer Plan Beneficiary Form to do so. 

Examples of Net Costs

Each plan option includes the Cancer Screening Benefit, which pays a benefit for each covered individual annually for taking certain tests, regardless of the cost of the test. Additionally, since the monthly premium is subtracted from pay before taxes, you receive tax savings.

Filing Claims

There is no deadline to file claims for the NCFlex Cancer Plan. Documentation is required.
