Starting January 1, 2023, the NCFlex Critical Illness plan transitioned from Allstate to Voya. Below are details for 2023. If you need details for 2022, review the 2022 NCFlex Enrollment Guide or contact Allstate (contact information is at the bottom of this page).
To file a Critical Illness claim for a diagnosis or a wellness claim, click "Plan Information" below to see flyers on the formless claim submission process. Keep in mind, for any diagnosis claims prior to 1/1/2023, these should be filed with Allstate instead of Voya. The Wellness benefit did not begin until 1/1/2023, therefore no Wellness claims are eligible on this plan prior to that date.
If considering the plan for the first time, here are a few highlights:
- Choose between $15,000, $25,000 or $40,000 lump sum benefit
- Pays a lump sum for major medical conditions at diagnosis including heart attack, stroke, cancer and many others
- Guaranteed issue – no health questions or evidence of insurability required
- Rates for you and your spouse are based on your age as the employee
- Unmarried children up to age 26 may be covered at no cost but must be added to the plan
- This plan includes a wellness benefit and COVID-19 rider starting in 2023
- Voya Financial administers this plan starting in 2023
