The State Employee Memorial Program honors State employees who lose their lives while in the line of service, or any other cause, and are actively employed by the State of North Carolina.

Upon the death of an active duty state employee, the employee's agency has the discretion to determine how they will implement the State Employee Memorial Program.  Links to the State Employee Memorial Program Policy and related forms are available below.
