Insurance If You Suffer A Loss
NCFlex offers two Accidental Death and Dismemberment (AD&D) Plans that pay a benefit if an employee or their covered dependent dies or suffers certain disabling injuries as the result of a covered accident while insured under the Plan. The coverage is effective 24 hours a day, 365 days a year and includes accidents that occur on or off the job.
The Core AD&D Plan is offered at no cost to employees. The Voluntary AD&D Plan offers additional benefits and the option to cover a spouse or family.

Coverage
The amount of insurance an employee purchases is called the principal sum. The maximum amount paid for losses from any one accident is 100% of the principal sum.
If an employee suffers any one of the losses listed in the chart, as the result of a covered accident, the loss will be deemed a covered loss and a benefit will be paid based on the percentage of the principal sum as listed.
If family coverage is elected on the Voluntary Plan, the principal sum for an employee's spouse is 50% of that of the employee. The principal sum for children is 10% of the employee's principal sum for each child.
If an employee and their spouse are both eligible state employees, both may participate but must choose to be covered as an employee rather than a dependent. An employee cannot be covered as both an employee and dependent. Similarly, children cannot be dually enrolled, so only one parent would be allowed to elect family coverage. In this case, the family coverage would apply to children only, since the spouse is already covered as an employee.
Employees of the University of North Carolina System are not eligible for this benefit.

Additional Information
- Loss of hands or feet means loss by being permanently, physically severed at or above the wrist or ankle.
- Loss of sight means total and permanent loss of sight.
- Loss of speech and hearing means total and permanent loss of speech and hearing.
- Loss of thumb and index finger means loss by being permanently, physically, entirely severed.
If an employee is actively working at age 75, the amount of insurance will decrease to 50%. Coverage cannot by increased once coverage is reduced by age.
Please note that coverage will not be in place during an unpaid leave of absence.
ReliaStar Life does not pay benefits for loss directly or indirectly caused by any of the following:
- Suicide or intentionally self-inflicted injury, while sane or insane.
- Physical or mental illness.
- Bacterial infection or bacterial poisoning.
Exceptions:
- Bacterial infection resulting from an accidental injury.
- The involuntary inhalation of gas and fumes and the involuntary taking of poison.
- Riding in or descending from an aircraft as a pilot or crew member.
- Injury suffered while in the military service for any country or government.
- Injury which occurs when you commit or attempt to commit a crime.
- Use of any drug, narcotic or hallucinogenic agent, unless taken as directed as prescribed by a doctor:
- which is illegal, or
- which is not taken as directed by a doctor or the manufacturer.
- The covered individual's intoxication. Intoxication means the individual's blood alcohol content meets or exceeds the legal presumption of intoxication under the laws of the state where the accident occurred.
Portability is an option for employees who want to continue their Voluntary AD&D plan, are under age 70 and are no longer active at work due to a leave of absence or separation from employment (including retirement). The premiums for ported coverage are the same as active employee premiums in the same age band.
